The agency requesting you to get the live scan needs to supply you with a “Request for Live Scan” form. You will need to bring this form with you completed along with a driver’s license or photo ID. As a courtesy the City of Monrovia has provided some common standard “Request for Live Scan” forms below; however, it is ultimately the responsibility of the applicant’s to ensure the correct form is supplied to the Live Scan operator

What is the Cost?
The City of Monrovia Human Resources Department charges a $20.00 rolling fee. However, dependent upon the type of Live Scan required other fees such as the DOJ, FBI, Child Abuse Index and Firearms background checks may vary. There are many types of background checks, or combination of checks and the fees will vary depending on the following:
- Requesting agency
- What type of check is required?
- The level of service and application type
All fees must be paid at the time of scanning and may be paid by cash, credit card, or money order made payable to “City of Monrovia”.

How long is the process?
Please allow at least half an hour for your appointment. However, the process time may fluctuate due to the following circumstances:
- Some people just don’t have good legible fingerprints, and it may take four attempts to obtain the best quality print possible.
- Do you have your Live Scan form? If so, is the form complete? The City cannot supply Live Scan forms for other agencies. The completed form will need to be brought in with you.
- If your driver’s license will not scan on the card reader, the information will have to be typed manually.
How long before the requesting agency receives my results?
Please allow three to ten business days, some agencies will take longer based on their arrangements with the Department of Justice.
Will I receive the results?
No, the applicant does not receive the results. If the applicant is interested in reviewing his or her record he must contact the Department of Justice for record review.
How do I follow up or who do I contact regarding my Live Scan prints?
The requesting agency and/or the applicant may contact the Department of Justice at (916)-227-4557 this is an automated system available 24 hours a day. You will need the following information:
- Your date of birth
- The 10 digit Automated Transaction Identification (ATI) number located at the bottom of the Request for Live Scan Form the ATI number always appears in the following sequence 1 letter – 3 numbers – 3 letters – 3 numbers
What if the requesting agency receives a rejection notice?
Fingerprints may be rejected for several reasons. The most common are characteristics with the applicant’s fingerprints that would make them difficult to read. This would include blisters, cuts, scars and calluses. If an applicant’s fingerprints are rejected on this basis, the originating Live Scan agency must re-take the applicants fingerprints at no additional cost. An application for Live Scan service may also be rejected if the applicant’s form is incomplete or improperly filled out. In this instance, fees would be charged to the applicant to provide them with repeated Live Scan service. It is important that the Request for Live Scan form be complete and accurate when presented to the Live Scan operator for service.
Rarely there will be an instance where an applicant's fingerprints are rejected because of error on the part of the Live Scan operator. If this is the case the originating Live Scan agency must re-take the applicant's fingerprints at no additional cost.
If the Live Scan service was provided by a different Live Scan agency, the agency resubmitting the prints has the right to charge their rolling fee.
If the requesting agency receives a rejection notice from the DOJ, you will need to be re-printed and your fingerprints will be resubmitted free of charge. You will need to make an appointment to be re-printed and bring your rejection notice with you to the appointment.
If my fingerprints have been rejected because they are difficult to read, how many times must I re-submit my prints?

We can come to you! Should you have groups of 10 or more to be fingerprinted, traveling services are available, but must be scheduled in advance. A traveling fee may be charged in addition to the fees listed above.
